Appointments
Please call for appointments in advance so that we may reserve a time for you. We will make every effort to be punctual. We understand that your time is as valuable as ours. If you cannot keep an appointment, please notify our office immediately so that we can offer that time to another patient. We ask for 48 hour notice of appointment changes to avoid charges on your account.
Reminder
It is our philosophy to stress the importance of routine dental care. This prevents both unnecessary emergencies and added expenses. As a service, we will notify you when it is time for your periodic checkup. Therefore, please keep us informed of any change of address or phone number.
Financial Policy
By reducing billing expenses, we are able to keep down the cost of your dental care. Please help us by paying for treatment at the time of your visit. If extensive treatment requiring several visits is planned, you will receive a written estimate for the proposed services prior to the start of treatments. For your convenience, several payment options are available.
- Cash, check, VISA, Mastercard or Discover.
- 5% cash discount for full payment in advance on major treatment.
- Care credit.
Insurance Policy
We are preferred providers for many dental plans. Ask for details.
Please have current dental insurance information at the time of appointment to include:
- Insurance company name
- Claims mailing address
- Phone number
- Group number
Dental insurances are not meant to be a "pay all." Please know that there will be a co-payment due at the time of each service. If we take assignments on your insurance, we feel that 60 days is a reasonable length of time for us to wait for payment from your insurance company. Regardless of insurance coverage, you will be responsible for all charges, and you will be expected to begin payments at the time the treatment is rendered.
Care Credit for Dental Procedures
We’re pleased to offer our patients the CareCredit® card, North America's leading patient payment program. CareCredit works just like a regular credit card, but without high interest rates or other costly penalties. You’ll get low monthly payments for healthcare procedures not commonly covered by insurance, including dental procedures. Plus, you can use your card over and over for follow-up appointments.
